Sometimes life can get a little tougher to manage on your own, whether that鈥檚 due to age, a health condition, or recovering from an illness or fall.

91制片厂County Council鈥檚 Adult Front Door service is here to make it easier to find the right support when it is needed the most.

Information, practical advice, guidance and signposting are just a click away, on our dedicated Adult Front Door portal, www.worcestershire.gov.uk/adultfrontdoor.

In addition, for those who need it, it鈥檚 possible to make a direct referral to community services, the Targeted Adult Support Team, or to be assessed for Adult Social Care.

Mark Fitton, Strategic Director for Adults and Communities at 91制片厂County Council, said: 鈥淲e want to make reaching out for support feel simpler, personal and reassuring. The Adult Front Door service is about helping people stay independent, connected, and supported as part of their community, as well as offering a first port of call for Adult Social Care.

鈥淎nyone aged 18 or over can self-refer easily through our website. Complete it in your own words, explaining what鈥檚 important to you and based on your answers, one of our advisors will be in touch to guide you to information, advice, and local services that best match your needs.鈥

It is also possible to submit your self-assessment form to 91制片厂County Council for further support and guidance.
